When I receive the form submission, all of the question are jumbled together with no breaks.
The problem you are experiencing is due to your e-mail client removing what it believes are extra line breaks. In you e-mail client (usually Outlook) you will see the text:
Extra line breaks in this message were removed. To restore, click here
Click the text and the message will be restored to its correct format.
