When I get my response to a signup, all of the above come back with zip code info inside:
City: 71037
State: 71037
Zip: 71037

I have redone all three in my form and resaved html, etc. Do I need to start over on this form and remake it all?

I will have our programmer look into the issue this weekend. Unfortunately however, the most likely solution would be to re-create the form.
