Form: Participant Info

When I try to add a new field:

Bicycle comment: Text Box

click next

Width 80

click next

Then following comes up:

Couldn't Execute Query 2

under URL: http://www.mycontactform.com/panel/newquestion3.php

It appears that portions of your form have been corrupted and this is why you are experiencing the issue. It is most likely due to the fact that you used quotation marks in the question:

Driving (or being a passenger on) an "Wave runner" (large 2-person jetski) (we warn against using quotes during the form creation wizard).

You might be able to fix the form by deleting the above question from the admin panel and then re-adding it without using the quotes.
