I have noticed on a page where I implemented the form, the radio buttons and check boxes are misaligned on my phone when I check my site. I am using a Samsung GS4 and the page is http://www.motion3media.com/small-business-design-form.html

I've also had to pad my footer links way below the form to show correctly on my phone, otherwise the form overlaps my footer links. If you can help with that too I would appreciate it. On a computer it works fine it just seems to be mobile devices.

My main concern are the radio buttons/check boxes. If I can't make them look good then I have to find another solution.

I am unable to get the link you've provided to load. This is likely a matter of tweaking your pages layout or the CSS used to layout the page.
