I'm having problems with my form on this page: http://ustc.homestead.com/volunteer.html. I've used this form in the past with no problems. However, for some reason, the width has become incredibly wide across the screen... I know I need to change something in the code, but I'm not sure where. Any help you can provide is greatly appreciated.

The issue is that you have set your last questions called 'Comments' to be 500 wide. This is stretching out the entire form. Try a more appropriate width, like 40.

You may also wish to change this question to a multi-line text field rather than the single line one currently used.

Both of these parameter can be changed under Edit > Form Questions.
