I'd like to create a form where people can vote for one out of a group of photos. Is this possible with MyContactForm.com?

Yes, but you would need to add the photos in an external HTML editor.

Unfortunately, this is highly dependent on what you are trying to do. I'd start by using radio button type questions and going from there. Your best bet is to experiment with it a bit.
