I've just checked one of my forms by filling it in myself - the Canadian request for graphic files. one of the compulsory fields is 'city' and i filled it in but the e-mail i received didn't show it.
I put that field there so that I would know which company was requesting the files if it was a company with branches with the same name in many cities. |

The form seems to have somehow gotten corrupted. Please go ahead and delete the form from the 'Form Management' panel and then rebuild it.
