MY form name is tv sign up,

The email I am getting in my mailbox has the text populated into the incorrect fields, I removed the alst 4 of the phone numebr for privacy. Also I asked for the company and title of company to be required Please help.

Delete all fields that are not populating correctly from the administration panel and then re-add them. Once you've done this, update the new code to your web site. This should solve the issue. If it does not, you'll need to rebuild the form.
