An answer to one question on my form shows up as the answer to many questions in my email even though I type out the answer to each question when filling out the form.

You will need to delete the question that is appearing in multiple positions from the administration panel, and then re-add it. Once you've done this, update the HTML for the form to your website and see if the problem is fixed. If it is not, you will likely need to recreate the form.
