Nick,
We have a premium membership and use the form for orders. Around 30-40% of the time when someone submits their order, they have an additional item added on - it's always the same item. Can you please let us know how to fix this?

There is nothing technical on our end that would cause this. It is most likely a situation where the individual using the form is causing the problem. Additionally, it is not possible to diagnose any issues you are having without more detail. This would include the URL of the form, the specific item being added, when it is occurring, etc.

Why would we post our URL to a public forum? We are a paying customer of yours. What is your email address for technical support?
It's clearly not the individual using the form that has a problem - they are simply using the generated form from your web site.
We are having a problem supporting our customers because of your technical issues. Why not support your own customers?

I cannot make a determination without seeing the form. If you are not willing to post the URL, then there is nothing I can do to help you. There is nothing in the scripts that would cause the behavior you are explaining. It is most likely a case of either someone inputting their form submission incorrectly, a problem with predefined fields, or some other external force.
Per the Terms of Service, support via email is not given.
