I have a form that's been working for over a year now, and now suddenly I'm only getting the first field's information and nothing else.

The puzzling thing is that when I test it myself, and test it on other computers in my office, the form works and all of the information comes through.

Does anyone have an idea why this would be happening?

This is most likely an issue with the either the web client or email client being used to check the forms. This is typically caused when the email client attempts to be intelligent and change the email format. Unfortunately, this often does more harm than good. Please check you specific email client settings.
