There is no image being displayed on my form. Instead there's text that says, "Image Verification Disabled."

I haven't touched the code in weeks! This is TERRIBLE for my business. Please HELP ASAP.

Your Premium Membership at one point was allowed to expire. This automatically disabled visual verification for all of your forms since the account was no longer upgraded. Now that you have re-upgraded, you need to login and re-enable the Visual Verification (under the Form Security / Anti-SPAM portion of the control panel).
