The final three questions on my form are all set up to have multi-line text boxes for filling in answers. Whenever someone enters text in the first of the three boxes, that information is omitted in the e-mail that is sent to me when they submit the form. Additionally, if they enter anything in the second box, when I receive it that information is shifted up and appears to be an answer to the first question of the three. Likewise, if they enter anything in the third box, it comes to me as an answer to the second question. The third question always appears to be unanswered, even if it was answered.

Any help would be greatly appreciated. |

In the absence of a response I just started from scratch and redid my form. Seems to have done the trick. Thanks anyway. |

I am sorry for the delayed response. This problem occurs (very rarely) when something within the database record for the form gets jumbled. You deleting the form and recreating is the way to solve the issue.
