Hi, a few questions if you don't mind

The following questions refer to you hosting my form, ie I do not load these on my servers

1. Can I upload my company logo (jpg file) to the form?
2. Is it possable to put a comment field above or below a question?
3. Instead of putting all the questions in a long list can you put them into two columns and host on your servers.
