I've set-up a form here: http://www.teamburnham.org/SignMeUpNow.htm. All questions have been set to "Required" and when I complete them all and his the "Submit" button, I get the "Error:" message basically saying that the T-Shirt Size field needs to be filled out. Can anyone help, the field is actually field out and I still get this error message.

The HTML for your form on the website is wrong and not as we have provided. Make sure to delete everything on the page (go through the actual HTML code and delete it as your HTML editor may miss things). Re-add the HTML as provided, and it should work.
