Okay .. I made the form .... I copied the HTML ... I posted it on the RIGHT page on my site .... and this is what I got:

I've fixed the problem. The problem you were experiencing was due to the fact that you left the form width blank when you were creating the form. I have put in some more error checking to ensure that the form width field is never left blank. For now, I have set your form's width to 500 pixels. You may change this if you want.
In the process of figuring our the problem, I fiddeled with the subject settings for your form. You will need to go in and edit the subject so that it fits what you want.
