Hello,

Well originally I thought I had this fixed but I was wrong.

I have been using My Contact form on several of my sites.

However when we installed this form,
found here http://www.livepersonality.com/web-actor-spokesperson-order-form.html and did several test messages we continually get a copy in my email that fields with incorrect answers displaying.

I am not modifying anything and pasting it in EXACTLY as shown.

In fact I have copied and pasted it in 4 separate times and still having the same issue.

This is what it sends me that is incorrect-

Billing Address: Walk-in on Screen from Left
City: Massillon
State or Province: YES - leave clickable from actor to LP

So basically the state field and address field keep pulling answers from fields that do not correspond.

I am very confused and see no reason why this is not working but need it corrected ASAP.

Could someone take a look at the code for the form on my site and tell me what the problem may be perhaps?

I would be most appreciative to have this rectified as quickly as possible. Feel free to test it if you like, as we also have it set up to where it sends a copy to person who fills out the form.

Can someone from management or other, please address this?

I would like to also sign up for the premium service (as I have done with 2 other accounts) but I need to know this form is going to work first.

I just went in and deleted the form in its entirety and started over from scratch.

Now it appears to work just fine....for anyone else who ever has this issue, it appears this may be your only solution.
