I have completed the form as per instructions but when I copy and paste into my website using Dreamweaver® or Contribute® for Mac there is no "paste html" so I used "paste text" then "options" which brings up a number of radio buttons none of which have "html". Consequently, I it won't tranform into the application no matter what browser I use. Is this a Mac issue?

My application title is TradeMaze's® Trade Mark Application form.

Within Dreamweaver paste your code into the HTML view for the site. The other alternative would be to paste the code into your page using a plain text editor.
