What are we missing? I did a test submission of form and it came to the email address I put in, however the FROM: is blank but I have the Subject

I know it has to be a simple issue. Just can't determine what i doing wrong.

Please post the form URL, as well as a sample of a message sent including the email headers.

Here is what came to me as the sample email:
FROM: IS BLANK
TO: Email address is my work place (don't want to show it here)

Thank you for registering for Conference 2006. The complete process for

your payment will take 3-4 business days. Please check your Paypal/bank or credit card

statement for confirmation. We will send you a confirmation letter via

mail or email with conference details.

It is being From Email is Blocked @ the workplace.
