Our customers have been telling us that they have been getting errors when submitting requests on our website. It is not constant, as they have told us if they try again, it works. I have tested all of the forms, and they work fine for me, but it think this is something that needs to be addressed.

The customers have said the error is something like "This form is not being sent from the url where it is supposed to be located. If you are the owner..."

I have made minor changes to the forms, like color, font, and table width, and made note not to change any of the identifying form codes.

Our forms are located at:
http://www.anodynetherapy.com/patients/already_request_info.html
http://www.anodynetherapy.com/patients/patients_find_provider_requestpacket.html
http://www.anodynetherapy.com/Contact.htm
http://www.anodynetherapy.com/professional/refer.html

Thanks!

The error you are receiving is correct. You likely do not have the Form Location setting configured with all variations of the URL that your form can be accessed from. For example, say your form is located at:

http://www.mydomain.com/index.html

This same page could be accessed through all the following domains, all of which would need to be added to the Form Location setting:

http://mydomain.com/index.html
http://mydomain.com
http://www.mydomain.com

You also need to check to make sure that there are no typos in the configuration. If you can get the submitter to copy and paste the exact error, it will tell you specifically which URL is missing from the configuration.

Login > Edit > Basic Form Information > Form Location
