It says not all the required fields were entered?

Potential causes for this error are:

1. The submitter of the form did not fill out all fields on the form that we marked as being required. If the submitter did fill out all of the fields that were marked as required, it means that there is a problem with the form (see #2 and #3).

2. The form has not been configured correctly. Login to the site to preview your form. If any of the fields in the preview have a red asterix (*) that means they are required and should be marked as such on the live version existing on your website. If a field is marked as mandatory and you don't want it to be, you need to edit that question. Please note that the email, send to (if it exists), and subject (if it exists) fields, are mandatory fields on all forms.

3. The form does not have the latest version of the html for the form. Click here for instructions on how to obtain the latest version of your form's HTML to paste to your web page.
