Why do I keep getting the following error on my form at http://www.reggae-boat.com/new/reservations.html?

I doubled-checked all the coding and populated all the fields, so I don't understand.

We're sorry but you did not fill out all of the required fields. Please push the back button on your browser and fill out all of the required fields.

I've suspended your account. Per the Terms of Service, users are not permitted to send credit card information or other sensitive data via our forms.
