Is there a trick to get the attach file to work? everything seems to work but no files come over in my email. I am using outlook express and I can get attachments from people when they email them to me. I copied the form to the following webpage in case you want to look at the code, but that is not where it is setup to be located as it will be behind a passworded directory.

Typically this is an issue with the email client being used. Have you tried using an alternative email client. Typically I prefer to use the free email client called Thunderbird.

Sometimes certain web hosts will not allow their user's up put upload fields on websites hosted on their service. You might want to check with them too. Also, have you tried creating a different form with an upload field and testing it?
