Good to be here, this is my first post.
Anyhow, is this the gist of Premium Membership?
a) For $24.00 per year I get to make forms for my website
and I can either use your server to host them, or my own
hosting.....is that correct?
b) If I use your servers there is a limit of 5,000 request per month....correct?
Questions:
a) If I use your server, what security do I have over my
incoming data?
b) Should I host the form at your server, will it look like the
rest of my website?

True, but the cost is $24.99/year.

This is true for both the hosted forms and regular non hosted forms.
Questions:

incoming data?
Basically the security available through myContactForm.com is the highest possible given the technology of email forms. The fact that the data is sent over email means that it is NOT encrypted in any manner, and therefore should not be used (and is prohibited per the Terms of Service) to send any sort of highly sensitive data such as credit card numbers or social security numbers. For more information, please read the Terms of Service.
b) Should I host the form at your server, will it look like the
rest of my website?
Yes, and no. The look and feel of the hosted forms are controled by the settings in the control panel. These settings are fairly simple compared to what can be done in an external HTML editor thus making it tough for the form's to precisely fit the look of you existing site.
