I am currently registered for your regular service and am curious what exactly the monthy form submittal quota is?

Does "form" represent responses from customers, or does it represent the number of say survey's I could create in a month?

The monthly submission quota is simply the number of times the 'submit' button can be pressed an any of your given forms over the course of 1 month. You may have as many forms as you wish, but the total number of submissions cannot exceed your monthly quota.
