Hi. I want this form uploaded to my blog. I entered the information into the HTML source. The form comes up and looks pretty good, despite some weird characters and a devil emoticon on the last two questions. When I tested the form myself and submitted it, the form seemed to reset itself instead of redirecting me to my homepage, as I had requested. Also, I am not getting any e-mails so I am pretty sure this isn't working.

Any help would be great.

The HTML is completely not correct. It appears that the CMS system you are using, is not showing the HTML correctly.
