This form was working until I made one small change in form management. I made sure to make the same change on the site...but now when you click submit, it goes to http://www.mycontactform.com/sendform/sendform.php and says "this page cannot be displayed"
Here's a sample of the form: http://www.farleydimmock.com/beta/index.html
I've tried starting a new contact form and simply cut and paste to a page, but it just comes up with this....argh.
Please help!

The problem is that you have significantly altered the myContactForm.com code. This is typically not an issue, but it appears that on your site you have moved the <form> tag outside of the <body> of the HTML page. This is why it is not working.

Hey Nick, thanks for the reply. When I move the tags back inside the body, it doesn't work either.
I've simply cut an pasted the code from your site on this page: http://www.farleydimmock.com/beta/testdelete.html where you can see the form tags are well within the body tags. It's not styled correctly to match my site but I just wanted to see if it would work. It still doesn't work...sends me to that same page.
Can you please help? Thanks in advance!

I gave up with that last form. I ended up using the host to provide the contact form.
Now i've got another situation with a host that does not offer that support (the reason I used this service to begin with)
Take a look:
http://home.comcast.net/~martincampersales/index.html
comes up with:
"Error:
This form is not being sent from the url where it is supposed to be located. If you are the owner of this form you need to ensure that the 'Form Location' is specified as http://home.comcast.net/~martincampersales/index.html. To change this, login to the site, click 'edit' next to the form's name, and then click the 'Basic Information' link to change the form's location.

Please do exactly as the error message is telling you. Add the following URL to the list of addresses that the form can be sent from (note the /index.html):
http://home.comcast.net/~martincampersales/index.html
