Can't edit form questions

Every time I click on 'edit' on a form question, it gives me the same question to edit (never the one that I clicked on). System is going to http://mycontactform.com/panel/edit_questions2.php (no variables in the URL).

The form I am working on is called NCAA-Update, the and question it continually displays is 'City'

Wasting my time getting same issue....

I click on edit to make changes on certain question an get the one above, I have to redo all question and answers all over.... Mad
