I have 4 forms created. When I test one form, the information being sent is based on another form.

I have reviewed all the setups for the forms and don't see any problems. Everything appears to be set correctly.

Any suggestions or is more information needed?

I am running out of quota for forms for the month and it is just starting!

It is possible that this could be caused if you have bits of an old form's HTML on your site. Go through your webpage and delete line by line all bits of any form code. Once you are sure that your pages are clear of all old code, re-copy and paste the proper HTML from the From Management panel.
