I tried updating my form through the "get html" link, but now there are no form fields next to the questions, with the exception of 2 questions. Any ideas on how to fix this? Thanks in advance for replies!

Matt,

I logged into your account, and both the get HTML and Preview areas seem to be working for me. If you copy and paste the HTML to your webpage, it should work. You may need to clear the cache on your web browser to see the updated changes (you can do this by holding the shift key and pressing the refresh button on your browser).
