When someone sends the form, it works in that I receive an email with their info, however THEY get a page that says cannot be displayed, which makes them start all over & re submit the form & believing it did not work.
I can't find any problems with my code, I believe my server uses coldfusion (if that helps) http://www.candidsbyalyce.com/contact.htm

The problem is that you have the redirect page set to:

http://www.candidsbyalyce.com/index

You can see that you are missing the .htm after index. You should set the redirect page to:

http://www.candidsbyalyce.com/index.htm
