I've tried to go into form mgt. and edit form questions to make sure name is on top but when I generate the html it is always Under email, can someone please help me figure out what I am doing wrong. I can switch the names in the html but the string input name="q[1]" type="text" id="q[1]" prevents "email" from being on bottom. Thanks in advance for any help.

The problem you are experiencing is most likely due to you telling the wizard to place the e-mail field at the top of the form. Verify the email question's location by:
1. Logging in to myContactForm.com
2. Click edit next to your form's name
3. Click edit questions
4. Click the edit button next to E-mail Address
5. Set the insertion for the e-mail field to something other than At the top
