Hi, I have been using this form for awhile now and it has always worked good. I was just notified that it is not working at this time. So I've tried it myself and keep getting this message even though everything is filled out properly..

"Error:
We're sorry but you did not fill out all of the required fields. Please push the back button on your browser and fill out all of the required fields.
Connection Type
Which games do you play"
You can try it yourself if you would like. The site is fmzclan.net. And you would need to click on the "join us" button on the left.

Any help with this problem would be appreciated.. Thanks..
