We are using MyContactForm for our online church prayer requests. We usually don't meet the 20 per month quota and use the free service. However, we have been recieving a repeat request, and we feel this is someone just playing with the form. How do we (if at all possible) block a certain email address from sending a contact form.

While it is not possible to block an individual from doing this right now, our next scheduled update will have features allowing you to block specific IP addresses from submitting your form, as well as limiting the number of times an individual can submit your form over a given time period. Please look for these changes within the coming month or so.
