Our Registration form no longer works. Users receive this message upon clicking "Submit"

There is an error with the identifying credentials for the form you are trying to submit. If you are the owner of this form you need to make sure to paste the html for the form exactly as it has been provided.

The code is exactly as provided but the form does not work. I even recreated the form with "fresh" code and receive the same message.

The form was working properly yesterday.

I tried a second form on the same site and received the same error message.

Please advise. Thanks
