I have followed the instructions to copy the code and paste it onto our website KMBABASKETBALL.org it isn't working. It is only showing the code not the form. Not sure what we are doing incorrectly.
Any help will be greatly appreciated.

Good Day,

What authoring software are you using for the site? Typically it is best to paste the code into the 'code view' for the site or do a 'Edit > Paste HTML' type function. These are pretty typical options in most pieces of editing software, but the language and procedure tends to vary.
