Radio and check boxes only display on standard web page but not an option on mobile view. Customers cannot select from items when we have them required as they don't see on option on the mobile version. We have checked the HTML fields and sample provided below:

Apologies for the delayed response. Typically this is an external CSS style sheet issue caused by the template of the web page the form is places in. If you could post a link to the form, I can confirm.

Thank you for the reply. Unfortunately, we cannot provide a link because we had to remove the form and recreate using a different service since we had customers trying to submit information but could not select the radio or check boxes and were receiving the 'error required fields notice' while accessing from a mobile device. We figured it might have been something to do with the web-page CCS style but were able to get all the elements to display properly using an alternative service. We may still use mycontactforms for other surveys and forms in our emails with clients but are not using on the website now.
