We are receiving duplicate forms. What could be causing this?

When someone is on our website and fills out a form, we receive the form twice.

Why? How can this be changed?

Please check to see that you have only one 'Send to Address' specified and that you do not have any sort of email forwarding occurring on your end. There isn't anything on our end that would cause this.
