My form shows up on my website just fine, but while testing it I filled everything out and clicked "Submit"...and nothing happened.

Re-copy and paste the html to your site. The HTML it is showing is different than the HTML we have provided (no form tag). It is possible that your hosting provider does not allow forms and is thus removing it without your knowledge, but I would recopy and paste first to make sure it isn't a simple copying error.
