Hello, I'm hoping to use mycontactform to send out a survey via email and allow users to submit their questions from their email client. Is there anyway that this can be done with this service? I tried but it's restricted because of the location of the form being submitted doesn't match the location on file.
Is there a way to do this? Premium membership, custom coding?
Please advise!

I'm sorry, but you would still need to use the form on a live hosted website (basically linked to from the email message you're sending out). I think that you could probably also host the form on a web server and then use an iframe to embed it into the html message you're sending out.
