On this page I have a series of button/rank questions.
First and last work and hold their "answer". But when you answer the second and then answer the third, the second answer "disappears" as if those two were sharing a "single response" piece of code.

I have not modified the cut and paste code at all. I pasted it as in on my template page -- which I have used with mycontactform for a long time now.

Thoughts?

Unfortunately, there is an issue with the backend database record for the form. It should be able to be corrected by:

1. Delete the 'Quality of Service' and 'Value for Money' questions from your form (Login > Edit > Form Questions > Delete)

2. Rebuild these questions (Edit > Form Questions > Add Question)

3. Update the form code on your web site
