My form looks great on both previews created on your site but when I "Get HTML" code and copy it to my site, some of the fields are missing and/or moved. Do you have any advice on how to fix this?

P.S. My site currently looks like this, but is laid out incorrectly (all fields below the ESTIMATED GROUP SIZE LINE):

Good Day,

It looks like something in your template is aletering the form appearance. I am unable to pinpoint the exact issue. I would try to generate the form using Div Tags versus the Table version currently on your site. This option is listed under the Get HTML page near the bottom where is says: "Layout Form Using".
