Hi !

I used the form wizard to create a Simple Web Feedback Form. I customized the HTML to suit our requirement but structure is the same. Now I need to capture the Windows Login User who will use this form.

How can I add that option onto this Feedback form ? or is there any other sample form that can provide this option

I am not sure exactly what you're hoping to do. It could be a matter of simply adding the fields your trying to capture?
