Hi
Can I create a form and place it inside of a php include?
I would like to have only one form and place it in a few locations on a site.
Before I attempt this I want to see if there is a best practices way of doing this.
My concern is that there will be a few different URLs used, and I know that form management requires that I list the URL of the page that the form will be on.

Yes this is entirely possible. You can either user a wild card for the form location setting, or completely get rid of it. The Form Location setting is not longer a mandatory configuration.
Login > Edit > Basic Form Information
