When I try to fill in the information in this form, it immediately redirects to my home page.

http://site.successtelevision.biz./emailteamsdvd.html

I have other forms on the site that work well so I can't figure out what the problem is here! Please tell me what I'm doing wrong.

Typically this is caused when you post the form code into an already open form tag on your site. Look at the HTML source code for your webpage that contains the form and make sure all form tags are closed before inputting the mycontactform code.
