The url is
http://originalpizzaone.com/PizzaOne/Contact.html
This is where the form is not sure what to use as I create the form
I have tried just the url originalpizzaone.com I have tried the above url
I have also tried the error message url that is given
Not sure what to do at this point.
Please help my client and I are frustrated.

Please do as the error message is telling you. The URL set for the Form Location configuration is incorrect. iWeb embeds HTML within an iFrame. This is why you are receiving the error.
1. Login to your account.
2. Press 'Edit'
3. Press 'Basic Form Information'
4. Set the 'Form Location' to:
originalpizzaone.com/*
www.originalpizzaone.com/*
This will cover all variations of the URL.

Please help! this is the message my clients receive when they submit a registration form..
Not Found
The requested URL /Mystage/Callboard.html was not found on this server.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

You are getting this error because you have set the form redirect page to a URL that doesn't exist. Under the setting Edit > Redirect Page please make sure that you enter a valid URL for the user to be taken to upon submitting the form.
