How do I give options for payment with check boxes please?
eg
1. Full: $200 by Dec 1
2. Concession: $150 by Dec 1
3. $20 deposit to secure a place

I'd like a check box for each of these options but not sure how to do it.

You'll either want to use the Check Box - Multiline question type or the Radio Button question type. If you need form submitters to be able to select multiple items, use the Check Box - Multiline question. If you want them to only be able to select one option, use the Radio Buttons question type.

During the wizard (or afterwards) you choose the question type in the same step that you are entering the text for the questions.
