Hosted forms are not enabled for this particular form.

When an upgraded account is allowed to lapse, it reverts back to being standard and all premium features are disabled. Since you then went ahead and re-upgraded after the downgrade had already taken place, you'll need to login to your account and re-enable the hosted forms since it was disabled when the account was downgraded. This can be done under the 'Xtras' portion of the control panel.
