I have the same problem that I see others addressing but the suggested fixes are not working. When I submit, I get the error message:

We're sorry but you did not fill out all of the required fields. Please push the back button on your browser and fill out all of the required fields.
Phone Number:

I've completely deleted the HTML a number of times and rebuilt it. Please help me.
