The pre-defined calendar field is not reflected at all in the form on my website although it is in the HTML coding. I've reviewed and attempted a couple of the corrective recommendations posted herein but still without success. Any help getting this to work properly would be most appreciated. Thank you.

There is likely a conflict with the calender and the CSS used on your site. Please post a link to the live form so that I may look at it.
